Insurance

Your dental insurance policy is an agreement between you and the insurance company. We will assist you in submitting the necessary forms. We do take assignment from your insurance company with prior arrangements. We require a pre-treatment deposit on large cases. No insurance company covers all dental costs. Some companies have fixed allowances for certain procedures, while others pay a percentage of the charges. It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid for by your insurance company.

Payment

We do not bill. Payment is requested at the time of service, unless prior arrangements are made. We accept cash, checks and major credit cards (VISA, MC, AMEX and Discover).

Your appointment time is reserved for you. Your time as well as ours is valuable, so a minimum charge of $25 per half hour of time will be made for a broken appointment if 48 hours notice is not given.
